docs(plans): roll 2.3.1 into 2.4.0; track the release doc 448f4c0ee9

Commits plans/releases/v2.4.0 and v2.4.1.md, which existed only as an
untracked working-tree file, with a scope note recording that the fixes
planned as v2.3.1 ship inside 2.4.0 (the implementing branches are cut on
top of this work; 2.3.1 never ships as a version number). The release
README's 2.3.1 row moves from 'ratified (pending implementation)' to
'folded into 2.4.0'.
